Insurance Information
We accept most major insurance plans and we are a Medicare provider. If we are not a member of your insurance plan, we will be happy to bill them for you.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
When you come to your appointment be sure to bring your insurance card, a list of any medications you take with their dosage, a list of any medications you are allergic to, and any relevant studies such as x-rays, MRI scans, etc.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals during our normal business hours, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances. All patients requiring narcotic pain medications must be seen in the office for re-evaluation at a minimum of every six weeks. This type of medical practice is focused on the diagnosis and correction of certain musculoskeletal disorders. Not all problems are amenable to rehabilitative or surgical correction. If this is determined to be the case and you are still having significant pain, we have professional referral relationships with several physicians/clinics who are specialists in the treatment of chronic pain and we will be happy to help you arrange consultation and treatment with them.
